Do you mean a form asking who has parental responsibility and who has legal access?

I just tagged the questions onto my child informatoin form along with... any allergies? Doctors details? etc.

hi , i rang ofsted about this and a nice man phoned me back :eek: well he said that we only need to know who can and who can not collect the child etc but i read it out to him and said thats not what the book says!! so think ill 4get what he said, and do what sarah is doing just to cover my back as another member has also mentioned this and inspectors have asked!!
