Hi, could anyone give me any pointers on what to include on a parent quesionnaire about whether they are happy with my setting?

Hi there,

If you look through the threads in this section you will get loads of ideas.
I have asked the following :

1. Are you satisfied with the service provided to you?
2. How flexible do you feel my service is, hours etc?
3. Is there any aspect of the service that you would like to see improved?
4. What are your view/equipment provided?
5. Are you happy with the methods that I use to communicate on a regular
basis?
6. What are your view on the safety and cleanliness of the premises?
7. What are your views on the food provided?
8. What are your views on the methods of discipline used?
9. What are your views on how your child’s individual needs are met?
10. What are your views on how al children are treated with equal respect?
11. Are you advised/consulted about any change to arrangements?
12. What does your child say about their care I.e. do you feel they are happy?

and then abit at the bottom, which they sign and date.

Oh my i am loosing the will to live! i've been trying to complete application 2, to become a childminder, all day! I have just completed the parents questionnaire and i am now blank for the childrens one. Can anyone tell me if i need to do a general one or one for all ages at this stage?
