New starters questionnaires

Can anyone point me in the right direction or willing to share (or Sarah do you sell these) a questionnaire for children (older) and parents to complete before starting. As in, what do you enjoy doing etc.

The Bromley one isn't give me any inspiration at all.

I've no idea where to get one tbh... which is why I'm trying to find inspiration myself. :rolleyes:

How close is it to being finished, might just wait and pre-order one but have a new starter waiting for one.
