permission to share information

Hi does anybody have a permission form to share information between parents and schools etc that i could adapt for my own setting? This would be greatly appreciated. Thank you

Yeh its quite hard to find, i thought bromley would have a guide as they have many others but not this particular one.:angry: x

Do we only do this for EYs??? I have 2 boys starting FE and I'm wondering what kind of introduction to do on their first day at school. For over 5s is a verbal introduction enough?
