View Full Version : Advice needed re: Child information

In my child info forms I have a section that asks for details of vacinations the child has had well a parent has left it blank and when I spoke to him about it he said she has all she should of had but doesn't know which. I am not happy about leaving it blank - what should i do?

I would ask the parent to put in

''all up to date''

I would get parent to put that too. After all it is what I put on my own children's forms:D

Should all be listed in childs red book along with dates.

I'd ask the parent to fill it in properly - it may be that the child hasn't had all or some of the vaccinations and I think you need to know that

When I used to work as a paediatric nurse and the parents couldn't remember we used to write in 'Up to Date' and that would be fine.
