permission forms

I am newly registered and just wondered if permission froms will be sent through with my NCMA start up pack. I have been told that NCMA provide them are these something I need to request. Does anybody use these I have been wondering whether to put something together myself.

I make my own.

I have permission forms for EVERYTHING lol, going in the car, taking photos, going on the trampoline, going on outings to name but a few!!!!!!

I just have a basic form which parents sign when they start with me and goes in their file.

I think NCMA might have examples, but you can do your own if you prefer.

I use my own as well... they sort of build up over the months as you hear about something else someone gets pulled up for on an inspection and you pop that into the next print run.

I'm sure there are some on the Ncma site... I got ideas from there a while ago :D

I make my own up but those on the ncma site are a good starting point.

Remember your paperwork should be relevant to you and your setting

Some childminders put it all on one sheet but I do individual ones and with the outings permission I added this to my list too.
