Health declaration address

Is the address to send the health declaration form just the normal Ofsted address? I have a letter from them asking me to send to the below address, with no address below. I don't want it to get lost!

I spent ages looking for the address too. In september it was the normal Ofsted address and as far as I know it hasn't changed :)

hi I would phone and check with them as I sent to normal address and it never turned up, had to get another one filled in and sent off to completely different address x

Just wanted to say, I called Ofsted to double check and it is the normal address.
