Form to record personal details etc?

I'm looking for a form that I can use/adapt for recording personal information....as in what new parents would complete so you have all contact details etc? I've searched and searched but can only find policies/consent forms etc. Not sure if one exists or whether people make their own up? Thanks. x

I'm going to get my Child Record Forms from Pacey xx

I have made up my own, it gives me everything I need and I can email it to parents so I keep it stored on my pc rather than in paper format too. It is on a passworded hard drive which is locked away so not readily available should anything untoward happen too.
