Hi,
I was just wondering if I have to have a formal form for parents to sign and return to me for my records advising me on updates when their children have injections or can I just write on forms one year old injections now been had.

There is no formal requirement to record anything about injections ourselves. But I would like to be informed, and some child record form templates include a question about if a child is fully vaccinated or not.
