Leaver's form

Hi,
I wonder if anyone can help please. I'm looking for some sort of feedback form for if a child leaves the setting. I can't seem to find one on here, I'm sure I have seen something like it somewhere.
Any tips what to put in it if I make my own up?

Just ask questions like "have you been happy with the care I have provided for X over the last ? years" "have you been happy with the outings and activities etc" "Have you found me to be approachable" Give them a space to write their own comments.
