Questionaire for old childminder?

Ve a new child staring that's leaving a old childminder due to them moving away.
Has anyone got a questionnaire template for old childminders to ask where the child is, what she enjoys there, how she is with other children and in different settings?

I would ask to see their Learning Journals and also ask for a review similar to the ones used for 2 year check ups - I always do this before my mindees start at Nursery so the Nursery knows what they like and don't like doing and at what stage of their development they are at.

Why not give her a ring for a chat (get permissions first) she might even invite you round so you cansee where the child has been and what he or she likes doing.

I find a chat on the phone and a look at the old LJ best. The LJ belongs to the child so should be coming with the child anyway.
