As and when childminding contracts?

I'm new to childminding and have been asked to look after a child as and when needed - no set hours or days per week. I know you need permissions signing etc.. but how do you fill in a pacey full contract? - confused 

What do you put in the days and hours part of contract?

I do it on the standard pacey contract but don't fill in any days/hours. I make a note that it's for ad hoc care with no guarantee to offer or accept an offer of work on either side. I back this up with my own booking form which contains additional T&C's for ad hoc bookings, with regard to getting paid before accepting the booking and what happens if they/I cancel the booking.
