Existing injuries

Hi :)

I'm a new childminder and need a pre existing injuries form. I don't really want to have to buy the pack from pacey as I already have accident and injury and medication forms. I am happy to make my own but wondered can anyone point me to a good body map I could use?

Morning! I have made up my own existing injuries form just wondered do I have to give parents a copy? Of so is it acceptable to scan it once signed and print for the parent?

Sorry another question!! I have got a child who is starting with me who has a medical condition and it causes bruise like marks on his skin I want to do a existing injury form for the child, however I collect and drop back at school so parent doesn't come to the house. Do I need to do the form with the parent or can I do it and just ask her to sign? I am including a body map to show where the marks are
