Health declaration form?

Hi all,

I printed off all my registration forms and all completed, however on the applicant checklist dated july 2012 document it stated that you need to send all forms to ofsted national business unit except the health dec form?...

So... Where am I meant to send it to? Have tried looking on the ofsted website and can't find anything.

Help much appreciated!

Health check goes to your doctor who will send it off when completed. Is that what you mean?:D

If you send off everything to ofsted they will reply asking for the health form and a cheque for your fees for the registration!

Our D O told us to send it ourselves so they didn't get lost?! But then nothing seems to have a structure for all with this process!

Yes thanks Sarah! Only my doctor called me to collect it so now it's in my possession in an unlicked envelope!
