Pacey contract help with the small print

Hi all, wondered if anyone could help, in the small print it says about the contract being linked to policies and gives you 2/3 very small lines so what do you put???

What I put is policy and procedure pack and the date, but also have a separate sheet with all my policies etc listed on that parent signs and ticks to say they have read all and understood. Don't know what others may do?
