These Development Journals are closely linked to Development Matters but they have been re-written specifically to help you meet the needs of children who are disabled.
The blurb says 'This Journal is particularly useful if you know or suspect that your child or a child who you are helping is unlikely to progress in the same way or at the same rate as other children - whether or not a particular factor or learning difficulty has been identified and given a name.'
I hope you find it useful :D

^^ link does not appear to work any more. This one takes you to an information booklet with further links - http://www.ncb.org.uk/media/884281/how_to_use_the_early_years_developmental_journalv2 .pdf
