Activity evaluation form - help!

Not sure if anyone can help - I am looking for a template/form for evaluating an activity but not an observation form iyswim? Basically I did an activity today - there were things that worked well, things that need to be changed etc and I want to record it somewhere. I use the Baby's Days system which has great obs but nothing to record something like this.
