Any risk assessment issues are noted in my diary...is this enough for Ofsted or should i include a specific form for folder to note issues raised and how they were dealt with? :idea:

If I have a RA problem I note it on the RA it is on and note what was done about it.

I would normally update my original RA - but sometimes if it is specific to the child I will note it in their LJ - or in my diary if it's a quick mention.
it depends on the circumstances and what works best for you :D
