Accident forms

Just wondering can anyone tell me, do you have to use the accident forms from ncma or can you do your own?

I have recently done my own on A4 paper and print off keep in a file :)

I use the NCMA ones for major incidents and a exercise book for minor scratches and bruises, 1 page for each child and then when full it goes into each child's file

I do similar, made a printed form for minor injuries as one particular mindee I used to have almost needed a form each day and I was worried where I was going to have to store all my accidents books as we have to keep until child is 21 - Also the NCMA ones are expensive!

I make my own and use them for all accidents/incidents.
