Hi

I have a child starting who does not have medication all the time, he only has Piriton if he has an allergic reaction to egg, dairy or pet hair. Do I need the parent to fill out a Long Term Medication form or just something that says to give him Piriton when he needs it?

And should they provide the Piriton or me?

I think I would fill out a long term form yes. Then you have their permission to give it. It is also there if they tell you he has had any in the preceding 24hrs so you can record when and what dose so it you have to give it there won't be an overdose.

Yes I would expect the parents to provide and I would sign a statement with them that you will inform them when it is going out of date so they can replace it.

I would use the long term medication form but make it clear that to be given "if needed".
I have an asthma sufferer, but he doesn't need it all the time so have done this for him.
Parents need to supply the medication and if it's prescribed then it needs to be in the original packaging with his name on etc.
