Booking forms

Hi, can anybody please send me an example of their booking forms, for holiday only care, please. Just got asked bu parent and not sure what to write and how to word it.
Also, how do you charge for holiday only care?? Do I charge my part time rate of £5.00p/h or more as its holiday only?? I know that I will take payment in advance with no refunds. Does that sound ok?? Thanks:laughing:

i would do a ad-hoc contract, stating day and times required, then i tend to charge a flat rate £25 per day to keep inline with other minders in my area and less than the nurseries :-) and i agree payment up front no refunds x
