Im looking to see if anyone has a form that i could use for parents to sign for their fees, other than the contract.

I have everything detailed on my Contract and I would have thought that was enough.

Why do you want another sheet as well have you had a problem?

I thought they had to sign for handing over the money and for me to keep a track of how much they have paid.

All you need is a receipt book to give them a receipt of what they have paid you. .Put the date amount for week/month and sign so they have proof of payment and so do you.You also put it in your account/expenses book.

I do a receipt and get the parents to sign it and I keep a copy. I use the NCMA receipts.

I bought a receipt book from Wilkinsons and fill out one for parents. I have a payment sheet which I keep with the attendance record and write down payments there and get parents to sign that monthly, which works well for me as most of my parents pay me monthly.
