View Full Version : Change of Surname-paperwork needed?

I have been informed that one of the children (and their mum and dad) has changed their surname, does anyone know what paperwork i need - will a letter from the parents or a copy of the change of name document be enough, or do i need to redo all the childminding documents - record forms,contracts etc?

My gut instinct would be to redo the paperwork in the new name.

I think I would phone the legal dept of my insurers for advise though.

with my daughter, the only thing needed was a copy of the deed change so school could add it to their records, I suspect this would be ok too, but i might draw up an additional sheet to add to the contract that stats that the child from such a date will be known by their ne name and that the contract signed on such a date still stands, and henceforth the child will be known as, and get parents to sign it.

Thanks for your advice, i have phoned the legal team and they are happy with a letter from the childs parents, am waiting a written reply from Ofsted but verbally they have said a letter and copy of the deed for change of surname is all i need.:)
