New starter Risk assessment

Do any of you do a risk assessment when a new child starts your setting. I give the parents a risk assessment form to fill in asking if they are confident with the stairs do they use a highchair do they have experience with dogs etc . Is thus enough or do you have a new starter risk assessment and what do you include
