Does anyone have a form for sending home to parents when a child has hurt themselves or bumped their head please

You don't need to send anything home... just write the accident report and when parents have signed make sure you give them a copy saying how the child has been since the bump and what steps tehy should take if they are worried.

I made this one. Like Sarah says, you don't have to have one, I'm just generally paranoid.
It's mostly nicked from the NHS website anyway.
