Feedback form from parents for Learning Journeys

Hi,

I was just wondering if anybody uses feedback forms for there Learning JOurneys. I use a A4 Book which has Look, Listen and Note sections, Photograph Observations and a Planned Observation once a month. I am planning to send the book back to parents half termly, but want to get their feedback on how they like the journal, what improvements could be made, feedback for development...etc even though I include feedback from verbal and a weekend book which I use to see what the child has done over the weekend.
Just wondering if anybody has got any ideas on what I could include as I need to do this on a seperate sheet as no rooom in the book.

I normally ask parents and write in their replies - use a different coloured pen - as they are dreadful at writing but happy to comment.
