Parent questionnaire

Hi all
I have ofsted coming this week to grade me and wanted to ask my parent to do a questionnaire abou care etc but havent got time to right one, i always seem to be running out of time, does anyone have one they could share?

rather than a questionnaire, as time is short, why not do a short comment box. Tell them that ofsted are coming and is there anything they'd would like to comment to ofsted on your service/setting?
