Does anyone have a holiday form that the parents fill in please that they wouldn't mind sharing?

Is it a holiday request/notification type form? I don't use one I'm afraid, I just ask the parents to tell me and I stick it on the calender.
I have a table I made to go in their folders that is basically just a record for me which details how many days they get, how many they have taken etc You can have a copy of that if you'd like?
