All about Me Forms

Hi,

So ive got a new starter starting with me next week and started printing out the forms for her.....and deciding on every form im printing off I dont like it. (These were the first attempt - not been registered very long)

So my question is does anyone have an all about me form they use for their new starters they love and would kindly like to share with me.

Also when they start, ive got the following paperwork;

Contract
New child record form
Permission Form
All about me

Is there anything else i need?

About your child form
Policy signing grid

I'm not sure if I need to include anything else. My form all about the child doesn't have many questions on it at the moment, just things like what foods they like/dislike and about their routine.
