Hi,

I was just wondering whether anyone uses these? I made my own which I find a bit more in depth but wondered what the general consenus was about these forms?

I do use these but have often wondered about doing my own to save on the huge cost! I wouldn't put anything else on them probably as I have other questions covered in an 'All About Me' booklet.

which leads me nicely onto the all about me booklet. Ive decided I want to update this too.

I was going to remove some of the duplicates but upon reveiwing my mindees paperwork at the moment it appears mum didnt put hayfever on one form but n/a and on the other form she put put 'hayfever'

Does anyone have an example of an all about me form? ive seen several but cant seem to pinpoint one I like.
