toilet training/being sick need advice please

hi really need some advice please ive looked after a lo for 3wks hes nearly 3 was in nappies but last week told me he needed a wee. today mum arrived and said hes not wearing nappies now but will have acident unless i keep asking him. well this morn he pooed on toilet brilliant well done lots of praise then he was sick kept retching it was awful. then this afternoon pooed in pants whilst i was cleaning/changing him he was retching being sick all over bathroom spoke to mum she said hes always been this way???? to me this aint normal any advice please i think it should be stopped but how????any one experienced this.

If a child is being sick he needs to go home for 48 hours exclusion.

If mum says the sick is linked to the poo then she needs to get him to the doctors urgently I think.

thanks sarah mum gunna take him docs as hes done it at home today so hopefully they will be able to help him
