definitive list for each child

Does anyone have a definitive list or any suggestions as to what each child should have in terms of paperwork?
Apart from risk assessments, policies etc. for the setting, each child has a file including:
assessments,
observations
initial assessments.
contract
emergency numbers
parental resposibility form
form to say parent has received policies inluding safeguarding.
parental questionaaire
medication forms
accident / incident forms.

Any further suggestions for my paperwork mountain?!!

Quite a few childminders do risk assessments for each child (as well as general ones for the group).
