    Default Obtaining parents feedback

How do you obtain feedback from parents without making it sound like you're after a huge ego stroke and loads of compliments about how great you are?! I worry that if I were to ask for feedback it looks like you're hunting for compliments so how do you ensure good honest helpful feedback to help you improve your service?

I keep questionnaire to one side, only 5-6 questions, can't remember exactly but I think mine are
Do you feel your child has settled in well?
Are you happy with activities/trips offered? Any suggestions
Are you happy with menu? What are your child's favourite meals at home at the moment
Are you happy with the overall service I provide? Any suggestions
Has your child mentioned an activity or trip they particularly enjoy or would like to do?
    Any further comments

I'm also going to put in there any changes to sleep/nap times/length etc as all mine are part time so often don't see the gradual change I would with a full timer
