    Default RA for self-awarding variations

I was going to design a sheet which allows me to quickly risk assess each time I need to award myself a variation under the EYFS and wondered has anyone else done this and would they be willing to share? I just want something quick and easy which I can keep each time to show I have thought about the elements involved in taking an extra child.

I just have a blank standard sheet and write above it, what is is for. Out and About, Living Room etc. So if it were me, I would print this out and title it Variation fill it out the usual way.
