Booking form?

Hi, has anyone got a booking form for me to use for some adhoc holiday care a parent has requested ?

hi, i just have a list of dates for the requisite holiday and ask the parents to write next to any date they require care for, the child (rens) name(s) and hours.

i also write on this form that all hours booked are to be paid for in advance. as soon as they give me the form, i then give them an invoice for those days to be paid within a set period of time. i say that once that period of time has passed, i can not hold those days and they become available unless they have been paid for. i also state that i do not refund or swap days unless i am unable to offer care for the requested day.
