I'd break it down and re-use parts to cover all kind of trips. You might cover:

    - leaving the house/keeping children under control near open front door/communal staircase etc
    - walking/buggy/reins/road safety
    - getting in car/car seats/seat belts/car safety/getting out of car
    - playgrounds/large play equipment
    - outdoor spaces/issues with dogs/keeping children under control in large spaces
    - how to make sure children don't get overly hungry/cold/thirsty/wet through
    - how you will deal with emergencies (first aid, what you will carry with you)
    - how you will be able to get help if needed (mobile phone, carrying contact info etc)
    - water/ponds/lakes/seaside

Then any very specific things for the park you have in mind (or the shop/soft play/farm etc etc).

You don't have to write everything down (actually, nothing HAS to be written) but at least think it all though and make enough notes to remind yourself so you can explain to an inspector and be confident you are able to keep everyone safe in all the eventualities you can think of.

In addition to Moggy's great advice, specific places are often able to provide you with a written risk assessment. For example I emailed to ask and got one from my local city farm, my nearest soft play centre, and the city zoo.
