Default could someone clarify what exactly is the hazard?!

I'm just doing my RA's and unclear what exactly goes under each heading.

    My columns are:

    1.What is the hazard?
    2. Who could this harm & how?
    3. What is already in place?
    4. Further Actions required?

If you take front door or smoke alarm for example would you do it:
Hazard : front door / smoke alarm
    OR
Hazard: front door not securely fastened / smoke alarm not working or testing routine in place

I much prepare the idea of nice bullet points for the hazards then detail in the other columns but all the templates I've come across seem to do it the other way!
Sorry if this is a really dim question

I identify the hazard so front door
then the risk: not secure, mindees escaping (coz I haven't managed to lock them in the cupboard yet)
Actions taken: check it's locked at all times with keys out of reach
    Further Actions: continual monitoring

A hazard is something that could cause harm to someone...

A penny dropped on the floor / sickness / an open drain cover / a broken railing at the park

A risk is what could happen to someone from the hazard...

Choking / cross contamination of bodily fluids / broken leg / escapes

A control is the step(s) you put in place to protect the someone from the hazard ...

A daily RA to check the floor for hazards / strict sickness policy and procedure if a child is ill / cover the drain / talk to children about safety or leave the park
