    Default CM2 form

I'm planning on registering my partner as my assistant and have been informed by ofsted that anyone applying to become an assistant must fill out form CM2 and be CRB checked. As the CM2 form and CRB checks are requirements for anybody living in the same house as a childminder, he must have already filled out the CM2 form in order for me to be registered so do I just put in a written request to ofsted stating this?

Give the help line a ring and ask. they'll probably tell you to just send in the variation request.

My variation only took 3 days to be confirmed last week!

Sometimes they want a new one anyway - it might be worth sending one in.

    Here's one to print -
