I have a 13 month old little boy who is teething, which part of the pacey medication folder do I get mum to fill out for Calpol to be given as a when required- not a form to be filled out on a daily basis.?

I use my own medication forms - http://www.childcare.co.uk/informati...dminding-forms - they are here.
I record parents wishes and they sign - I then write down when I have given medication and they sign again to confirm they are aware.
