    Default permission forms

    Hi all

    can anyone tell me what needs to be in a permission form... probably a really silly question but i've never actually wrote one before...

    I've got
    To seek medical advice / attention
    Take photos
    The use of plasters, suncream , napoy cream
    To use garden equipment, such as paddling pool , climbing frame etc
    To leave child with my registered assistant
    To take on trips / outings
    H4H supporter 'per mare per terram'
