Parent Questionnaires..

Morning all,

I've been minding for about 4 months now and my very helpful husband has suggested I get feedback from my families about how I'm doing etc... Nice idea but not sure how to do this..

Do any of you give out questionnaires to parents to ask how you're doing/what you could do better etc? If so, could I either have a look at one or could you let me know what sort of things I should be asking.

The paperwork just keeps on coming!
