Help with risk assessments for outings

I'm really stuck with how to do my risk assessments for outings. I've done all my general risk assessments, it's the ones for specific outings that I'm stuck on. I was wondering if anyone would mind letting me see an example of one just so I can get some ideas please?

If i am going to somewhere like a theme park or something, they should already have their own risk assessment so i get them to send me one of them and then do my own, after visiting the park myself, but i do the outing risk assessments along the same lines as the routine ones.
