Permission Form for Prescribed Medication

How should the Permission form for prescribed medication look like?
I had interview today with a mum who has daugther with astma and i had provide her with all the permission form and just realized that I should have permission form for prescribed medication and dont really know what to write on that kind of permission.and I wan't to make it general so if I get child with diffrent medication I won't have to write new one.
Any ideas??Pls

and do i need to add some information to be completed by physician like: Name of medication
Reason for medication
Form of medication/treatment
Restrictions and/or important side effects
Special storage requirements

or just simple parent permission with the name of the medicine is enough???

I personally use the NCMA duplicating acc/inc book, and would advise that if you are that unsure it is probably worth buying it from them or MM if they do one, just so as you can be sure you have everything covered for such an important thing.

I am stiil waiting for my certificate but it shouldn't take long form know because i have recived my CRB two days ago so don't really know what forms i will be getting from my insurnce company.
