    Default Medicine permission form

    Hi everyone

    I have attached a copy of a permission slip I have to get permission
    for myself to give any medication to a child in my care inline with my medicine policy.

    Do you think this is ok? Or do I need something else?

    Thats exactly what you need
    Then another form to fill out when you have given it - I got pulled up for not having a form like yours on my inspection

    hi, that should do the job im sure!!!! i have put my permission to give medicine and medicine administered on the same form, here it is here if you want a lool xx

    Thank you didnt realise I had to write when it had been administered aswell but I guess that makes sense

    Hello
    Just saw this as I am looking to simplify what I currently use. Your form is great Carlen. Can I just check though, do we not need parents and childminder to sign after each dose or is that just the NCMA forms being OTT?

    This is the one i use ,( hope it works , never put an attatchment on before!!) 2 inspections ago i was told that the parents had to sign before to give permission and after to acknowledge that it had been given , so i amended the origional NCMA form and the inspector was happy with that

    Hi, just to add I have parents state what medication has been administered within 24 hrs incase they forget to tell you in passing. (Prevent overdose)

    yes, on the form the top section is the parents telling me exatly what and when to administer, they then sign and date this, the bottom part is the bit i fill in when and what i gave then they sign again on collection to say they understand this is what was given, if you know what i mean. i do also ask if when the last dose was given just incase of overdosing xx

    I just heard that an inspector asked for a minder to change her policy on medicines to include what she would do if a mindee refused to take their medicine?

    I hadn't thought of that one! Not sure what to put - any ideas?
