afterschooler questionaire

hello!

i have a new afterschooler ( and fulltime in holidays ) starting in a months time. mum has told me what he likes to do, but i thought it would be good to get his view on things! ( as i have a suspicion that mum is telling me what SHE would like him to do/not do !!!

)

i've never had a questionaire for afterschoolers to fill in themselves before, and i've had my current afterschoolers for a couple of years now! so i know all about them!

apart from name ( ! ), school/teacher, food likes and dislikes, favourite activity, what else should i be asking? i can't think, and i'd like to give it to him next week, when he comes to play for a hour in half term! ( as i'd like him to do it on his own ! (or at least, he can answer and i can write answers!)

any ideas?

favourite board games, what does he do with his own parents at the weekend?, during the holidays if he could chose an activity/outing what would it be??

Not sure what age your after schooler is, but these are the forms I give out for the CHILDREN to fill in.
