additional needs information

Hi i have a child starting soon who has cerebral palsy and other medical condition's. is there an additional form available anywhere that i should fill in before she starts. i am trying to put one together but i don't want to miss asking any vital information of it. so i was wondering is there a form available i could check against to ensure i have all the information needed before she starts

You will need a Care Plan, made together with parents. There are various templates online.

Have you got any support from your Local Authority - they may well have a template they can send out to you.
