How do you recored who has Parental Responsibility?

    Default How do you recored who has Parental Responsibility?

    I have been informed that as I am with MM but do not have their child record forms I need to get parents to sign to say who has parental responsibility for the child. (Theres a section apparently on MM child record form??)

    Anyway - as I said I do not use this form & so not sure how to word it on yet another document for parents to sign???

    Any ideas?

    Parental Responsibility and Legal Contact
    Under the Early Years Foundation Stage (EYFS), which came into effect from September 2008, I am legally required to establish who has Parental Responsibility and Legal Contact of every child in my care.
    What is Parental Responsibility?
    A married couple who have children together both automatically have parental responsibility.
    Parental responsibility continues after divorce.
    Mother automatically has parental responsibility.

    Where the parents are not married the father has parental responsibility if:
    His name is registered on birth certificate (after 1 December 2003).
    He later marries the mother.
    Both parents have signed an authorised Parental Responsibility Agreement.
    He obtains a Parental Responsibility Order from the court.
    He obtains a Residence Order from the court.
    He becomes the child’s guardian.

    Legal Contact is anyone who has applied to a court for access to the child and been given legal contact rights.

    I get parents to fill in a pack of forms, including an 'All About Me' form, Medical details form, permissions forms, policies receipt form and a 'Next of Kin' form as attached. I know it's probably overkill but this way I can send it home, get parents to go through all the forms in their own time and bring back to me. I then fill in the contract and child record form myself and get parents to check it. Most parents write 'N/A' on the second sheet of questions but then at least I know it's N/A!
