Urgent Help re short-term contract

Hi,
A friend today asked me to look after her two children on Sunday morning under my "childminding hat"

I have said yes, but my problem is, is that i am only newly registered and have no short term contracts (they are in the post!) This is just a one off while the mum runs the Race for Life and only for a maximum of three hours.

I have spoken to Ofsted and NCMA who said that it is fine for me to look after them without having an official NCMA contract but advised me to draft my own. (i've spoken to the mum and she is happy for me to make up my own contract) Can anyone help me with what i should but in it as i havn't seen a contract so i have no idea.

I am seeing her tomorrow afternoon for her to sign a contract, permission forms and advise me of any allergies etc

Childs name , address, date of birth, emergency 2nd person and telephone number, doctor's address and telephone number, how many hours you are minding for and parents signature. Also dont forget find out if the child has any allergies yes or no still write it on the paper. Hope all goes well.
