    Default Do parents have to have a copy?

    Do parents "HAVE" to have a copy of form for things like medication give/accidents/incident etc?

    The reason I ask is I am re doing my forms so they are all "uniform" IYSWIM and I currently use the NCMA ones, but want to use my own. So just wanted to know really...

    Also if parents do have to have a copy does it have to be an exact copy, so print out twice? Write twice? Or can it is a small snippet I can cut off the bottom of the form stating something a long the lines of "Informed of incident/accident/medication on X date"

    YES PARENTS have to have an exact copy esp if you give medication etc
    the ncma one is good not seen the MM paperwork but if you do your own it muxt include the same info and have an exact copy that shows exact signatures of yourself and parent..

    It is good practice - not a requirement of the EYFS.

    The EYFS 2012 does not actually state that forms have to be signed! Just that parents must be informed as soon as possible.

    However you wouldn't have any proof they've been informed if you didn't ask them to sign so that will stay good practice as well.

    This is sort of what I had in mind... So I will still fill in a form and get parents to sing but just tear/cut off the bottom to give to the parent so if ever they wanted to refer back to the "event" then I would have them all in a file in date order.
