    Default Policy & procedure signing form

    Me again!!

    Is anyone happy to share their P&P confirmation form? As in the one parents would sign to confirm they have received, read & understood policies?

    Mine is literally only one line at the moment which looks silly and I dont think it says everything it should say to be acceptable

    Mine has my letter head on the top with my phone numbers and email.

    Then a fill in line for the child's name and date of birth.

    Then a title 'CONSENT FORM'

    I give consent for my child.... Then I list all the things I want consent for and at the bottom it states: I have read and received a copy of this Consent form.

    I then list all my Policies with the heading 'CHILDMINDING POLICIES' with:

    I have read, agree to and have received a copy of the Childminding Policies document containing the Policies listed below.

    I then have 'COMPLAINTS PROCEDURE'

    I have read and received a copy of the Complaints Procedure

    Then to end :

    The consents, policies and procedures are subject to annual review. The date of the next review is shown below. They may be reviewed earlier should the need arise due to change in circumstances, ofsted regulations or legislation. Your comments and feedback are always welcome.
